Pro and Contra: Do we still need Angiography? – FEOph Symposium and Roundtable Discussion

In the FEOph Symposium, junior speakers from France, Great Britain, Germany, Italy and Spain are holding lectures with focus on „Angiography“ with special emphasis on OCT-angiography. The presentations are followed by a roundtable discussion with five experts from these countries who will discuss the importance of dye-angiography and OCT-angiography vs OCT alone in the different national approaches and strategies.

Prospective study of 243 eyes from 125 consecutive age-related macular degeneration patients, showing a high rate (68.8%) of reticular pseudodrusen with an original multimodal imaging protocol. Sensitivities and specificities of each imaging modality have been calculated. Spectral Domain-Optical Coherence Tomography, Infra Red, and MultiColor had the highest sensitivity (99.3%, 84.6% and 87.1% respectively), and specificity (100%).

Background: In contrast to fluorescein angiography the Angio-OCT offers the opportunity of imaging the perfusion of the retina without injection, thus not invasive. Particularly for vascular retinal diseases like retinal vein occlusions (RVO) or diabetic retinopathy (DR) this technique seems to be very promising. Methods: We retrospectively analysed data from 13 eyes with DR or RVO comparing the results of Angio-OCT imaging compared to common routine diagnostic tools. All patients were examined using fluorescein angiography (FA), spectral domain OCT (Spectralis, Heidelberg Engineering, Heidelberg) as well as Angio-OCT (Angiovue, Optovue, Fremont CA, USA) in addition to ophthalmoscopic examination and fundus photography. The results of the different types of retinal vascular imaging were compared. Results: There was a high correlation between the FA and the Angio-OCT findings. Non-perfused areas were demarcated in most cases with both imaging techniques. Also shunts and IRMAS were visible in both methods. Microaneurysms (MAs) however often could be found more distinctly in FA whereas deeper located MAs sometime were better visualized by Angio-OCT. The vascularization of proliferations could be shown in Angio-OCT in more detail because of the fact that leakage is not present. Conclusions: The diagnosis of retinal vascular diseases is of particular importance. Angio-OCT, a non-invasive technique, allows to analyze not only the superficial but also the deep retinal perfusion. Macular ischemic areas can be detected as well as by invasive FA. But in contrast to FA, Angio-OCT does not provide information regarding retinal barrier properties and peripheral alterations. Longitudinal studies are needed to analyze the clinical importance of this promising new diagnostic tool for the monitoring of retinal vascular diseases.

OCT Angiography allows CNV detection by showing the blood flow without leakage or staining. This non invasive procedure give the possibility to perform several examinations in order to follow closely treatment results. Software may enhance type 1, type 2 and type 3 CNVs. We applied on selected patients an experimental software that shows CNV area and flow, in relation with the intravitreal injections and visual acuity. Our preliminary impressions were that CNV flow decreases 24 hours after injection and increases again after 4 weeks. CNV area decreases 24 hours after inj and re-increases after 4 weeks. CNV flow and area seem related to CNV activity. Vision is not directly related to CNV activity. Same main vessels seem to reappear after 4 weeks. With OCTA it is possible also to observe the morphological modifications of new vessels after repeated treatment.
